I have carefully read over Elkford Skating Club's Communicable Disease Policy and I agree to abide by the rules and follow the procedures outlined therein.  Furthermore, I will ensure that my skater(s), and any caregivers, siblings, friends, and guests of my skater(s), will abide by the rules and follow the procedures outlined in the plan. 

I understand that the ESC Communicable Disease Plan has been put in place to ensure the safest return to skating possible, and a failure to follow these rules by myself, my skater(s), or any siblings, friends, guests, or caregivers of my skater(s), may result in a termination of our club membership and my skater(s) being removed from their skating program, at the discretion of Elkford Skating Club.
